Overview & What is PCNL?

Kidney stones get formed in the urinary tract when chemical compounds crystallize in urine. PCNL (Percutaneous Nephrolithotomy) is a specialized technique used to safely extract kidney stones from the upper ureter or kidney when they are too large for standard ureteroscopy or shock wave lithotripsy.

The percutaneous nephrolithotomy procedure is ideal for patients with larger kidney stones. Through a small skin incision of around 1 cm, PCNL allows the surgeon to extract stones of 15mm or larger. For smaller target areas, a refined approach called MINI PCNL may be recommended.

Procedure Details & Postoperative Care

How PCNL Procedure is Performed

The surgery requires general anesthesia. To begin, the surgeon performs a cystoscopy using a thin, lighted telescope to inspect the ureter and bladder. A fine catheter is placed on the stone side to navigate up to the kidney.

Depending on patient positioning (prone or supine), a small incision (8-10 mm tract size) is made over the back near the kidney area under real-time X-ray guidance. An endoscope is passed into the tract to visualize and fragment the stones. A JJ stent is typically placed to aid healing.

Postoperative Care & Stent Tips

Be mindful of these common post-op signs and contact your doctor if needed:

  • Occasional vomiting or nausea can occur shortly after surgery.
  • Mild pain or urinary frequency in the first 24–48 hours is manageable with medication.
  • Light pink to reddish urine is normal; contact emergency care if severe bleeding occurs.
  • Ureteral stents are small tubes kept for ~14 days for swelling control and are removed at follow-up.

Benefits & Potential Risks

Major Benefits of PCNL

  • Minimal Postoperative Pain
  • Preserves Kidney Function & Reduces Tissue Damage
  • Keyhole Minimally-Invasive Approach
  • Significantly Lower Risk of Urinary Tract Infections (UTI)
  • High Clearance Rate for Large / Complex Stones

Risks and Complications

While PCNL is safe and routine, all surgical approaches carry potential risks to discuss with your surgeon:

  • Bleeding: Significant blood loss can occur, comparable to open surgery safety profiles.
  • Infection: Low risk when proper post-op antibiotics are taken consistently.
  • Organ/Tissue Injury: Slight possibility of adjacent organ contact (spleen, liver, lung, bowel, or gallbladder).
